How To Apply For Food Stamps
WHAT YOU NEED TO APPLY FOR FOOD STAMPS
Send (or bring in) COPIES of the information listed below. Never send
the original documents to your local office.
- ONE of the following forms of identification:
1. Driver’s license
2. State ID card
3. Voter registration
4. Birth certificate
- If you or any household members are Legal Permanent
Residents,
you need Alien Registration Numbers for each (green cards).
- ONE of the following items to prove Social Security numbers for
all members of your household(everyone you live with who is applying for food
stamps):
1. Social Security Card (or receipt showing that you’ve applied
for a replacement)
2. Any document with the Social Security number and signature or
Social Security number and photo of any household member who is
applying
3. Copy of birth certificates with Social Security numbers on them
- ONE proof of monthly housing expenses, such as:
1. Rent receipts
2. Letter from your landlord
3. Mortgage statements
- ONE proof of income for each member of your household who works, such
as:
1. Most recent month’s check stubs
2. Letter from employer indicating amount per pay period
- Proof of all non-work income (if you receive SSI, Social Security,
unemployment, etc.).
- Bank statements for checking and savings accounts
and proof of any assets
(IRAs, large amounts of cash, stocks, bonds, etc).
- Proof of medical expenses not covered by insurance for anyone in
the household who is disabled or over 60 years old.
- If a member of your household pays or receives child
support, include a cancelled check from the most recent month or other
proof.
- If a member of your household pays for child care or adult dependent
care, include a cancelled check/money order or a letter from
provider stating how much you pay and how often.
- If you or your children have a medical card, include a copy of the most
recent card you received.
