Establishing Community Quality Councils

Piloted in the following offices:

• Cook County
• Englewood—Region 2
• Oakland—Region 2
• Hoffman Estates—Region 1
• South Suburban—Region 3
• DuPage County Office

Coordinators: Donna Clay, IDHS & Sr. Connie Probst, IHC

Mission: to meet the needs of our clients by developing state and community partnerships.

Goals:

• Develop relationships
• Develop outreach plans
• Increase understanding and access of Community Resources
• Discuss trends and impact to a local community
• Increase understanding of policies and practices of IDHS
• Proactive problem solving

Meet quarterly
Designate a chair and secretary for the group
Notice to all one week in advance with time, place and agenda
The initial meeting is to build relationships among the attendees.

There would be one meeting in each area before January 31, 2004.
